Recertification Application Form

If you have not received your recertification application pack, this may be because we have your incorrect postal or email address. If you have changed either of these, please contact the IBLCE office to notify us. Please be sure to include your full name and ID number in the email. You can also download an application for the countries listed below.

IMPORTANT NOTICE FOR IBCLCs DUE TO RECERTIFY BY EXAM IN 2012: IBLCE would like to apologise.... The online system for submitting applications to sit the 2012 exam has been held up by some technical difficulties. In the meantime please print off your application from the appropriate link below and send it to the IBLCE office in Australia (postal details are on the Application Form).

Please Note: Your application MUST be complete at time of submission. If your application is incomplete you will be charged the appropriate deadline fee for when all documentation is recieved and an additional incomplete application fee applies.
